Kindergarten Registration

We are still accepting registrations for the 2016/2017School Year. The following items are necessary to bring to complete the registration forms:

  • Proof of Birth Date: Proof of birth date must be a legal birth certificate, a duly attested baptismal certificate, a passport or a hospital record.  In order for a child to enter kindergarten, he/she must be five years of age born on or before October 1, 2008.
  • Student born between October 2, 2008 - December 2nd 2008 qualify for Transitional Kindergarten.
  • Proof of Immunizations: You must show proof of immunization, such as the yellow California Immunization Record, to register your child.

Children entering kindergarten should have:

  • 5 DTP
    4 doses meet requirement if at least one was given on or after the 4th birthday
  • 4 Polio
    3 doses meet requirement if at least one was given on or after 4th birthday
  • 3 HepB
    at any age
  • 2 MMR
    1 dose on or after 1st birthday 1 dose on or after 4th birthday
  • 1 Varicella
    If your child had chickenpox disease, ask your doctor to note it on the immunization record to meet the requirement
  • 1 HepA
    recommended
     

 


  • Proof of Physical and Oral Health Assessment: Examinations administered after March 1,2011(within six months prior to entering kindergarten).
  • Proof of Residence/Address: Students must reside within the boundaries of La Merced School.  Residence is defined as the place where the student and parent/guardian physically reside on a regular basis.  This may be verified by two of the following: (1) rent receipt, escrow or mortgage papers, (2) utility bills (no phone bill) addressed to the parent or guardian at an address within the district, (3) voter registration certificate, (4) valid California driver’s license/identification.
  • If applicable: Any court order or legal documents related to student

1st - 5th Grade Registration

At the time of registration, you will need to provide the following verification of information:

  • Proof of Birth Date:  Proof of birth date must be a legal birth certificate, a baptismal certificate, or a passport.
  • Proof of Immunizations:Immunizations must be up-to-date.  A letter from you doctor or clinic listing dates of immunizations or an official immunization record is acceptable.

5 DTP

4 doses meet requirement if at least one was given on or after the 4th birthday

4 Polio

3 doses meet requirement if at least one was given on or after 4th birthday

3 HepB

  • Transfer:Paper work from previous school, and last report card

 

Your child’s class assignment and teacher will be sent to you before the start of school.
